What will I receive?
The Adult ADHD Provider Appointment Toolkit is a 15-page digital PDF. Choose Printable to write by hand or Fillable to type your answers. On this Shopify store, each purchase currently includes one selected paper size: US Letter or A4. No physical item is shipped.
Where is my download?
After your order is processed, check the email address you used at checkout for your download message. It may arrive separately from your order confirmation. Check Spam, Junk, and Promotions folders as well.
If the message is missing or a download link does not work, email hello@cognaaura.com with your order number and checkout email address. Please contact us before purchasing again.
Printable or fillable: which should I choose?
The Printable PDF is intended for printing and completing by hand. The Fillable PDF contains form fields for typing and selecting answers in a compatible PDF reader. The toolkit content is the same; the way you complete it is different.
How do I save my fillable answers?
Download the file to your device and open it in a PDF reader that supports fillable forms. Save a working copy before you begin. Enter a short test answer, save, close, and reopen the file to confirm your reader retains your entries before completing the rest. Keep an untouched original for future use.
If a preview does not let you type or save, download the file and try a compatible PDF reader instead. Contact us if you need help opening the file.
How should I print the pages?
Use paper that matches the file you purchased: US Letter is 8.5 × 11 inches; A4 is 210 × 297 mm. Start with Actual Size or 100% scale and check one test page. If your printer clips an edge, use its Fit to Printable Area setting. You can print only the pages you need.
Can I reuse the toolkit and share it with my provider?
Yes. You may keep personal copies, reuse the toolkit for your own appointments, and share your completed pages with your healthcare providers. Please do not resell, publicly upload, or distribute blank copies.
Who is this toolkit for?
The Adult ADHD Provider Appointment Toolkit is for adults already diagnosed with ADHD who want to organize information and questions for their healthcare providers. It is not an ADHD screening or diagnostic tool. These educational and organizational resources do not replace medical advice, diagnosis, treatment, or individualized care.
How do I request help or ask about an order?
Email hello@cognaaura.com or use our Contact page. Include your order number, checkout email, the format and paper size purchased, and a brief description of the issue. For technical problems, tell us the device and PDF reader you are using.
Please do not send completed toolkit pages, medical records, medication details, passwords, or payment-card information. A cropped screenshot of an error is helpful if it contains no sensitive information. This store's support channel is for product and order assistance, not clinical advice or urgent care.
