Product descriptions and delivery
Before purchasing, review the listing for the format, paper size, price, and files included. Products are delivered digitally; no physical item is shipped. Use an accurate email address at checkout so you can receive order and download information.
Printable PDFs are intended for printing and handwriting. Fillable PDFs require a PDF reader that supports the file's form fields and saving completed entries. Our Download Help & FAQ page explains how to check that your reader saves your answers.
Personal-use permission
Your purchase permits you to download and keep copies for your own personal use, print pages for your own appointments, and share your completed pages with your healthcare providers.
The purchase does not transfer ownership of CognaAura's original content or design. You may not resell the files, publicly upload them, distribute blank copies, or use them as a product for sale. Contact us before distributing copies to clients, patients, a class, or an organization.
Educational scope
CognaAura tools are educational and organizational resources. They are not diagnostic or screening instruments and do not provide medical advice, diagnosis, treatment, or individualized clinical care. Buying or using a toolkit does not create a nurse-patient or other healthcare-provider relationship.
The Adult ADHD Provider Appointment Toolkit is intended for adults already diagnosed with ADHD. Discuss health concerns and treatment decisions with a qualified healthcare professional. Do not delay seeking care because of information in a toolkit.
Protecting your information
You decide what information to enter in your downloaded copy and with whom to share it. Store completed files securely. Please do not send completed toolkit pages, medical records, medication details, passwords, or payment-card information through our store contact form or support email.

Order problems and refunds
